Mental Health Support Services
Supported Housing Services Fayette Resources Supported Housing Program is a comprehensive housing and related support program designed to provide adults with mental illness the opportunity to secure and maintain housing within the community. Services provided by this program are personalized based upon the needs of the individual to ensure that each person is given every opportunity to live as independently as possible. Functioning as one critical element among a carefully coordinated effort, this program is designed to work in conjunction with other services provided to the individual, including case management, counseling and vocational training. Each person referred to the program is assigned a housing specialist who assists with most housing and independent living issues. The services provided include:
To meet criteria for admission to the Supported Housing Program, consumers must be at least 18 years of age with a documented mental illness of a serious or persistent nature. Priority for admission is given to those that are a part of the C/HIPP Program or those who are returning to the community from a state or psychiatric hospital facility. The Specialized Personal Care Home Our Specialized Personal Care Home! Community Residential Rehabilitation Facility at Fayette Resources offers a program that enables people receiving mental health services the opportunity to live in a community setting while maintaining access to the focused care they need. These individuals receive comprehensive services, including mental health and adult daily living skills training. The agency staff develops treatment plans and outlines goals intended to increase each person's independence. These steps are taken to empower the individuals to live in a less structured environment. Family Support Services The Fayette County MH/MR Program recognizes that families play the most significant role in planning, implementing, evaluating and monitoring services which best address their own specific needs. Guided by the Family Advisory Council (FAC), Fayette Resources assists in a voucher system to provide funds to family members with developmental disabilities. |
